European accident form

Does anybody have, or know where I can get, one of the self duplicating forms to be used in case of an accident in Europe? The idea is in case of a burmp both parties details are on the document along with sketch of incident/damage etc.

 I used to get them with the car insurance in the olden days when I asked for a ‘green card’ but my supply is now exhausted.
